Forklift Driver Charged in Fatal Accident

Peel Regional Police have charged a forklift driver following a fatal industrial accident in Mississauga Monday morning.

Police responded to a call in the Kennedy Road and Courtneypark Drive East area, shortly before 9:30 a.m.

At the site, police determined a man was struck by a forklift. Police say the victim was pinned between the forklift and a flatbed truck.

A 42-year-old employee was taken into custody in connection with the fatal incident, police say. Police allege he fled on foot after the incident.

Police have charged the man with failing to remain causing death. A bail hearing is set for Tuesday.

Man Killed in Delta Industrial Accident

A 36-year-old man is dead following an accident at a packing plant in the Tilbury industrial area of Delta Tuesday afternoon.

According to Delta police, the man sustained serious injuries when he was run over by a forklift at around 2:30 p.m. Emergency responders were not able to help the man, who reportedly died at the scene.

The accident happened at the Cratex Industrial Packing plant on 92nd Street near the Alex Fraser Bridge.

As the incident is not considered criminal in nature, police said WorkSafe BC will now investigate the cause of the accident.

The man will not be identified publicly.

1 Killed in Springfield Forklift Accident

One person was killed in a forklift accident in Springfield early Tuesday morning.

The incident happened at Hail & Cotton at 815 Bill Jones Industrial Dr.

The 911 call came in around 2:30 a.m. Tuesday.

According to Smokey Barn News, the victim was trying to put something into a dumpster when the forklift tipped over on its side.

The victim’s name has not been released by officials.

Woman killed in Forklift Accident at QVC

Florence County Coroner Keith von Lutcken is investigating a workplace accident Thanksgiving Day at QVC Distribution Center on TV Road in Florence that claimed the life of a Timmonsville woman.

Lutcken said Agnus Ann McKithen, 45, was 30 feet in the air on a forklift when it  was hit by another forklift  inside the plant.

He added McKithen’s forklift overturned, causing her to fall.

Lutcken said she died at the hospital.

McKithen’s family said they’re devastated by the accident.

We’ve placed a call to officials with the SC Occupational Safety and Health Administration to learn if they’re investigating the accident.

Customer Dies after Forklift Accident at Goodwill Parking Lot

A customer was hit and killed by a forklift while walking through Goodwill’s parking lot in Lynnwood.

The 69-year-old Bothell man was on his way to the Goodwill in the 4000 block of 198th St. SW in Lynnwood around 10:30 a.m Monday.

“He was struck by the forklift, which knocked him to the ground, and then the box fell off on top of him,” said Det. Brian Jorgensen with Lynnwood police.

The driver of the forklift saw a flash of blue go in front of the forklift and he slammed on the brakes, said Jorgensen.

The victim was transported to Harborview Medical Center with life-threatening injuries and succumbed to his injuries early Tuesday morning. The King County Medical Examiner’s office told KIRO 7 Tuesday night that a death report had been filed for the victim. Still, KIRO 7 had learned the man was being kept on life support at Harborview Medical Center.

Lynnwood police detectives will continue to investigate the accident.

We asked Seattle Goodwill spokesperson Katherine Boury whether the accident was preventable.

“I think that that’s one of the things we’re looking into,” said Boury.

She says all of the company’s forklift drivers are all trained and the company is saddened by the tragedy.

But when we asked her about the driver — and whether he’s still on the job — she said she can’t comment on an ongoing investigation.

Police said they will be watching Goodwill surveillance camera footage as part of their investigation.

 KIRO 7 learned that Monday’s accident is the third forklift-related death in Washington state in the past few months.
